1. Membership
Wellness Rewards is a yearly membership offered by Foothill Animal Hospital, Inc., 29040 Portola Parkway, Lake Forest, CA 92630. One membership covers your whole household and all of your pets.
2. Price and term
The membership is $140 per year, paid once at enrollment for a 12-month membership year. Because it is paid once for the year, there is nothing to cancel. When your year is up you may renew at the front desk or by phone.
3. What's included
- Two (2) exams each membership year.
- One (1) nail trim each membership year.
- 5% back on every dollar spent at Foothill Animal Hospital, loaded onto your virtual card as rewards money usable towards future services.
4. Rewards money
Rewards are loaded to your virtual card automatically and can be used like money towards services at Foothill Animal Hospital. Rewards have no cash value outside the hospital.
5. Giving back
For every dollar members spend, Foothill Animal Hospital donates 1% to the Veterinary Care Foundation to help local pets in need.
6. Not insurance
Wellness Rewards is a membership, not pet insurance, and does not replace insurance coverage.
7. How to join
Enrollment is by phone at (949) 380-1255 or in person at the front desk. There is no online enrollment.
8. Changes
